If you have a health plan not listed below, you may be responsible to pay “out-of-network” prices based on the details in your plan. Contact your health plan for more details. Please note that insurance companies sometimes offer a variety of health plans with different coverage, eligibility requirements, and in- and out-of-network benefits. Accordingly, the list of accepted insurances below is not a guarantee that we accept your particular plan. You will be asked to present your insurance card at each visit so we can verify your insurance coverage and benefits and that we accept your particular health plan.


Insurance plans are negotiated between insurance companies and employer groups; therefore  not everyone’s medical coverage is the same.


We may require co-insurance, co-pays, or deductibles. These amounts are required to be paid at the time of service, as per your insurance carrier contract. Please bring your current insurance card to every office visit so that we may verify your insurance coverage.

Co-pays are a contract between you and your insurance carrier. Some contracts require more than one co-pay on the same date of service if you have two different types of exams or procedures done.

Prior Authorizations
Your insurance may require pre-approval for certain types of medications, specialist referrals and other procedure or testings.

Insurance Forms
We will complete one set of insurance forms for you. Please allow 7-10 business days for completion of disability, compensation or insurance forms.

Family Medical Leave Act (FMLA) Forms
We are able to complete one set of FMLA forms for you. Please allow 7-10 business days for completion of these disability or compensation forms. Fees may apply.
